    considering massage therapy as an added health benefit.

    To provide you with the best service possible, we ask all
    clients to fill out our Client Intake form. If you would like to
    fill out the form prior to your visit, please download the
    form below. Please bring in your completed form as we will
    need your original signature on file. There is a section for
    insurances purposes only.  If it does not apply to you, then
    please leave it blank.  Completing the form prior to your visit
    is not necessary, we offer it as a convenience.
